Improving End of Life Care in Care Homes across Yorkshire and Humber
Skills for Care are developing a programme, which consists of a number of workstreams, aimed at improving end of life care in care homes across Yorkshire and Humber.
These workstreams are divided into five different sections which are:
Manager development training programme
Managers from 800 care homes across the region will take part in the training programme, with the most fundamental national occupational standards (NOS) forming the basis of it. The training programme will be supported by an accompanying guide for care homes to achieve national end of life care quality markers.

Champion development training programme
Nominated champions from 800 care homes across the region will take part in the training programme, with the most fundamental national occupational standards (NOS) forming the basis of it. The training programme will be supported by an accompanying guide for care homes to achieve national end of life care quality markers. The champion may be the manager, deputy or senior care worker.

Development of networks
Networks will be developed to support care homes to share best practice, learn from each other and access further end of life care support and advice e.g. from hospice staff, end of life care facilitators and other multi-disciplinary team members. The ultimate aim will be for these to be self-managing.
Commissioner/regulator/contractor development
Training will also be available for those who are responsible for the on-going monitoring and measurement of care quality by developing their knowledge and understanding of end of life care. The training will be supported by the development of an associated guide to end of life care quality assessment and monitoring.
E-learning
As the work programme rolls out and care home managers and champions put their end of life care learning into practice, the learning and development needs of the wider care home team will emerge. In order to meet this need, e-learning modules will be available to make training both easily accessible and cost effective for care home staff.
Desired outcomes
All of the work streams are anticipated to achieve sustainable outcomes for care home residents that include:
- an increase in the number who have advance care plans in place
- an increase in the number who die in their place of preference
- an increase in the number who are allocated a key worker as they approach the end of their lives
- an increase in the number who are entered onto an end of life care pathway.
They are also collectively anticipated to achieve outcomes for care home managers and owners that include:
- a reduction in the number of resident and family complaints associated with end of life care delivery
- an improvement in their ability to demonstrate compliance with care quality measures.
For commissioners, they are also collectively anticipated to achieve outcomes that include:
- a reduction in the number of unplanned hospital admissions for residents in the last year of their life.
