Supporting your NQSWs during their ASYE and recording their progressive assessment is an important part of the ASYE. Below are the old ASYE templates (pre 1 September 2022) for those working in adults services.
The record of support and progressive assessment (RSPA) and the critical reflection log (CRL) are key evidence templates for recording and evidencing the ASYE assessment process. You should refer to the guidance for completing the RSPA and guidance for completing the CRL when completing these templates.
The following information details the responsibilities of the NQSW and assessor at each assessment stage and the provides the templates for the evidence that is required.

Part 1: Beginning the ASYE
Responsibilities of the NQSW:
Identification of learning and development needs at the start of the ASYE to inform the discussion at the support and assessement meeting; planning for the assessment process and the personal development plan (PDP).
Resposibilities of the assessor:
Setting out and agreeing the support and assessment agreement, including roles and responsibilities and developing the PDP based on the NQSW's reflection in part 1 of the critical reflection log.
